Zions CTMG5 is a club register with 200 tear out membership passes, where each visitor signs in as either a Members Guest or a Temporary Member on the same slip.
Registered clubs usually see two kinds of non member at the front desk: a guest signed in by a member, and a visitor from further afield or another club entering as a temporary member. Rather than keep two books, this register puts both categories on every pass. The visitor prints a name, date, residential address and postcode, then ticks the category that applies.
The Members Guest panel carries a short declaration with spaces for the guest signature, the member signature and a membership number. The Temporary Member panel has its own declaration, a signature, the club name and a membership number. Beside them a Welcome to Our Club panel sets out printed conditions for the visitor to read before signing, including that the slip is carried on the premises and is valid for the day of issue only.
