Zions No. 68P is a club time sheet for permanent employees, headed Weekly Employees, that records a full time or part time staff member's week from hours worked through leave to net wages, in a pack of about 100 sheets at 240 x 205mm.
Permanent staff bring things a casual sheet does not have to handle. Alongside the seven day start and finish grid, this form has lines for annual leave, annual leave loading, sick leave, other leave and a public holiday falling on a rostered day off, as well as Saturday and Sunday extras, overtime, public holiday overtime, shift penalty, a no break line and meal, shoe, tool, clothing and laundry allowances.
A tick area near the classification line marks the employee as full time or part time. The notes panel prints the part time loadings used on the form, and separate boxes take a part time hourly rate or a full time weekly rate.
In the pay section, gross wages step down through income tax to a sub total, then add untaxed annual leave loading and less other deductions to reach net wages. Deduction lines cover a voluntary Club Plus contribution, union, medical benefits and other. Calculated by and checked by lines let a second person sign off the figures.
