Each form is set out for ordering from a supplier. Under the Order heading and the form number come date, To and From lines, then the printed instruction please supply the following items, quote the above number on invoice. A Qty and Description table takes the order itself. The foot of the form has an I hereby quote ABN line with room for the authorised person to print their name and sign.
Asking the supplier to put your order number on their invoice is what makes the carbon copy useful later: when the bill arrives, it can be matched to the order you kept. The cover reads With Extra Carbon, and the carbon sheet sits behind the page you are filling in.
