A roll of 500 green triangular 25mm 'Extremely Thick' food consistency labels, printed with the numeral 4, on paper with a removable adhesive.
Level 4 Extremely Thick covers food and drink that holds its shape on a spoon and does not pour. It sits at the boundary between a drink and a puree, and it is one of the levels where getting the consistency wrong carries the most risk, so the label earns its place on the tray. Avery states the range was developed in accordance with the International Dysphagia Diet Standardisation Initiative framework, and the green colour and the pointing up triangle match the IDDSI convention for Level 4, so a tray marked with one of these reads the same way in any kitchen that uses the framework.
The label is green with black print, which is the combination IDDSI specifies for this level, and the numeral 4 appears on the label face so the level is legible without relying on colour alone. The adhesive is rated from -5°C to 60°C and Avery lists it as safe for refrigeration, so it holds from the cold room to the ward.
